what is the procedure to post an unblock request?

My character has been blocked since March,09 for no apparent reason.I want to return.

btw, i have not done anything illegal, never battled with multi in same battle, nor used money to transfer. just used to log on from multiple networks or locations, back then.
I have had made donations in diamonds that time.( i understand that doesnt make me immune,but still have not really done anything wrong :S)
