I have that problem to sometimes, it seems like the shift button gets tied to chat instead of the combat.
For me I can reset it by Ctrl+left click on a unit on the ATB bar. After that the shift works again. Or at least it works for me... |

Don't think there is one, I would classify it as a bug (I can report it in the ICC)
Workarounds:
f5 (takes some time)
Ctrl+left click on a unit on the ATB bar
Alternative method:
Hold left mouse button down for 3 sec until attack icon change (time needed can be changed in settings) |
