Can the appa's phantom be missed 4 times in a row?
Yes, doubly incorporeal.
It can miss upto 4 times, but the 5th hit will strike.

Incorporeal:
Physical attacks against this creature are only 50% likely to be successful. However, there is no way that three consecutive hits or three consecutive misses take place on every particular stack. After two consecutive hits a compulsory miss will occur, and vice versa. Incorporeal does not concern magical attacks against this creature. Any magic (except the "Raise dead" spell) applied to this creature also resets the consecutive hits & misses counter to 0.

Zeno is referring to an apparition - Which has incorporeal - and a summoned phantom of the version.

A phantom incorporeal can be missed up to 4 times. 2 misses for the incorporeal and 2 misses for the phantom.

Simply put. The answer is Yes.
From Mohini's example of 3 misses, we extrapolate that they can have 4 misses in a row, but has anyone actually seen it?
